- asyn-thrdd.c: scope an include.
- apply more clang-format suggestions.
- tidy-up PP guard comments.
- delete empty line from the top of headers.
- add empty line after `curl_setup.h` include where missing.
- fix indent.
- CODE_STYLE.md: add `strcpy`.
